Our role as the nation's largest and most experienced Financial Management Service provider is to assist those eligible Medicaid recipients to choose and pay for their own support workers and services within their state-approved personalized budget.

Responsibilities:
Manage a caseload of assigned program participants or individuals enrolled in the program, ensuring successful program participation and quality level of service.
Maintain required contact with assigned participants including in-person home visits and telephone contact.
Assess individuals in caseload for ability to successfully participate in a self-directed employer and/ or budget authority service model.
Make recommendations for placement of resources and/or corrective action if and as needed.
Identify and maintain available community resources and provide person-centered, need based community resource referrals based on eligibility criteria.
Manage follow up procedure post referral to track success and outcomes.
Provide program education and guidance to caseload, specific to individual choices, goals, and desired outcomes.
Educate caseload on provider roles and responsibilities for participation in self-directed services, including how to navigate program rules, expectations, and Financial Management Systems.
Provide individuals with practical skills training in areas including independent living, locating/securing community resources, recruiting, selecting, hiring, training and supervising employees, budgeting and purchase of goods and services.
Provide train-the-trainer instructions on, evaluate caseload for, and ensure compliance with identification and reporting of fraud, waste, abuse, neglect, and exploitation.
Independently support enrollment activities.
Facilitate successful development of service/ spending plans and establish a strong foundation for future spending activity in accordance with rules and constraints of applicable program rules.
Identify, report, and appropriately follow up on allegations or reports of suspected fraud, waste, abuse, neglect, and exploitation.
Maintain documentation of services provided and time committed in accordance with applicable policies and procedures.
Other duties as assigned.
Required Skills:
Ability to demonstrate initiative, manage schedule, and work independently with minimal direct supervision.
Flexibility and versatility to support individuals of diverse backgrounds.
Strong proficiency in Microsoft Office applications.
Ability to exercise discretion and decision-making judgment in assessment of participant needs and abilities.
Strong verbal and communications skills.
Must maintain appropriate etiquette and customer support techniques, including use of program and population-specific language and terminology.
Effectively prioritize, multi-task and meet required deadlines in a dynamic and fast-paced environment.
Must recognize and maintain confidentiality of all materials in the work setting.
Ability to maintain positive working relationships with federal, state and county agencies and other community stakeholders.
Other Qualifications & Requirements:
Bachelor's degree in related field preferred.
1 year of related experience, including knowledge of Medicaid services and work with individuals with disabilities and/or senior citizens, is required.
Related experience as Self-Direction Supports Broker, Financial Consultant, and/or Case Management experience strongly preferred.
Additional education and experience requirements may be required and will be indicated by state contract requirements.
Background Check A valid driver's license, access to a motor vehicle, and active vehicle registration, inspection and insurance certification required.

The base pay for this role is $40,000 - $47,600 annually; base pay may vary depending on skills, experience, job-related knowledge, and location.
